NCPH event sponsors and cosponsors will be recognized in the printed program if the NCPH SPONSOR REGISTRATION FORM is received by November 1, 2011. Submit by February 15, 2012 to be recognized in the Onsite Guide. All event sponsors and cosponsors will be recognized with signage at the Annual Meeting.
